IV. EMPLOYEE BENEFITS
A. Health Insurance
College Division and Pre-College Division faculty members who teach ten or more hours per week are eligible for health insurance coverage on July 1, or as soon thereafter as it is determined that ten hours will be taught. Each eligible employee has the opportunity to select from two Oxford Health Plans. These plans combine managed care coverage and traditional insurance coverage into one plan. Both Oxford Plans offer the flexibility to use the physicians/providers of the employee’s choice. When services are obtained through the Oxford Primary Care Physician, Oxford HMO benefits will be received. If an out-of-network physician is used, payment will be made in accordance with the indemnity schedule of benefits. Members can check claims, benefits and referrals; request materials or ID cards; change physician; and search for doctors and specialists by logging on to Oxford’s website at www.oxhp.com.
A portion of the cost of health insurance coverage will be deducted from the paycheck. The type of coverage selected will determine the amount of the deduction.
B. Dental Insurance
College Division and Pre-College Division faculty members who teach five or more hours per week are eligible to participate in a dental plan through Aetna (formerly Prudential). If the employee elects this coverage and teaches 10 or more hours per week, the employee and the School each contribute 50% of the monthly premium for individual coverage. Faculty members who choose to cover dependents pay the cost of the family portion in full. Faculty members who teach between five and nine hours per week can enroll in the plan but must pay the entire cost of this coverage for both the faculty member and his or her dependents.
Medical and Dental coverage is available for the faculty member, his/her spouse or domestic partner and children.

C. Group Life Insurance and Accidental Death and Dismemberment
College Division and Pre-College Division faculty members who teach ten or more hours per week are covered on the first of the month following three months of employment. The amount of coverage is equal to the employee’s annual salary, rounded up to the nearest thousand. Premiums for these plans are currently paid in full by Juilliard.
D. Long Term Disability Insurance
College Division faculty members who teach ten or more hours per week are covered on the first day of the month following one year of employment. If the employee is totally disabled for more than 26 weeks, this coverage pays a portion of the salary. Premiums for this plan are currently paid in full by Juilliard.
E. COBRA
Under a Federal law called COBRA (Consolidated Omnibus Budget Reconciliation Act), full-time College Division and Pre-College Division faculty members have the opportunity, in the event of termination of employment, to choose continuation of Juilliard’s group health plan identical to the coverage currently being offered, for a period of 18 months. The faculty member is required to pay the premiums in full for this continuation of coverage.
F. Pension
College Division faculty members who teach a minimum of 300 hours per school year are eligible to participate in a Pension Plan through TIAA-CREF after two years of service. If the faculty member’s teaching hours fall below 300 in any school year, no contributions will be made by Juilliard to the pension account for that school year. Employees who already own a TIAA-CREF individual retirement annuity contract are eligible for participation on the first day of the month following employment, provided they teach 300 years for the school year. Service with a prior Eligible Employer may be used toward meeting the two years of service requirement of plan participation. Juilliard contributes 10% of the faculty member’s College Division salary. Employees are also eligible to contribute a certain percentage of salary to the annuity contract through salary reduction (pre-tax), which reduces salary for tax purposes.

G. Supplemental Retirement Annuity - SRA
Faculty members who are not eligible to participate in the Retirement Plan, as well as those faculty members who already own a TIAA-CREF individual retirement annuity contract, are eligible to reduce their salary by a certain percentage (pre-tax) in order to make contributions to a Supplemental Retirement Annuity. Please see the Benefits Coordinator for further information.
H. Workers’ Compensation
All Juilliard employees are covered by Workers’ Compensation, an insurance carried in case of a work-related illness or injury. It is essential that every injury or accident be reported at once to the Office of Human Resources so that the proper forms can be filled out. Please report even those injuries not requiring immediate medical care. If medical care is required, physician and/or hospital bills should be sent to: PMA Management, 5789 Widewaters Parkway, Dewitt, NY 13214, Policy # 6282123. DO NOT pay the bills directly to the provider and DO NOT submit bills to Oxford.

I. Tuition Remission
Children of faculty members who teach ten or more hours per week are eligible for full tuition remission in the College, Pre-College and Evening Divisions. All required fees must be paid. Children of part-time faculty members are eligible for a pro-rated reduction in tuition based on the number of hours taught. Further information can be obtained from the Business Office.
